They go with it. An extension's entitlement comes from its parent licence, so refunding the parent removes the add-on's entitlement too.

Yes, on the parent product's update entitlement. When the parent's entitlement lapses, the extension stops receiving new versions along with it — and, like the parent, it keeps working.
